If you said “yes” to both questions, we’re inviting you to take part in a
brief survey to share your opinion about guide dog use. Five individuals
who leave their email address at the end of the survey will be randomly
selected to receive a $20 Amazon gift card.

The American Foundation for the Blind is pleased to be working with Guide
Dogs for the Blind on a 21-month research study to examine guide dog usage.
